Notice of Data Event

We are notifying individuals of an incident that may affect the privacy of certain personal information relating to former and current employees, patients, residents, and other individuals. We are issuing this notice to communicate what is known about the incident, our response, and steps impacted individuals can take, if deemed appropriate.

What Happened? Earlier this year, we learned of potential suspicious activity in our computer network and promptly took steps to secure our systems and initiate an investigation with third party cybersecurity specialists into the nature and scope of the issue. The investigation determined that an unauthorized actor gained access to specific systems and copied certain files within our network from December 5, 2025, through February 12, 2026 and viewed or copied certain files. As part of the investigation, we undertook a thorough review of the potentially impacted files to determine whether any sensitive information was present. We completed our review on July 1, 2026, and are notifying potentially affected individuals.

What Information Was Involved? The categories of information that may be affected will vary by individual, but generally include name, address, date of birth, driver’s licenses, passport card number, medical treatment information, diagnosis, prescription/medication information, and treating/referring physician.

What We Are Doing. We take the confidentiality, privacy, and security of information in our care very seriously. Upon discovery of the event, we conducted a diligent investigation to confirm the full nature and scope, took prompt steps to ensure security of our network environment, and conducted a comprehensive review of the information potentially affected. We also notified law enforcement and enhanced existing security protocols.

What You Can Do. Although we are unaware of actual or attempted misuse of any information, we are providing notice of this incident out of an abundance of caution. As a precautionary measure, we encourage individuals to remain vigilant against incidents of identity theft by reviewing account statements, credit reports, and explanations of benefits for unusual activity and to detect errors. Any suspicious activity should be promptly reported to one’s health care provider, insurance company, or financial institution.
